Service Coordinator (Immediate start)

Job Profile:

At Greenleaf Care Services, Service Coordinators work as part of a team to ensure that services are delivered in accordance with Greenleaf's vision and values and the National Disability Insurance Scheme

As a Service Coordinator you will be an integral part of Greenleaf’s accommodation service delivery. As House Coordinator you will be responsible for the day-to-day operations of your allocated houses (4-7 houses) and care needs of the participants in your houses.

Duty Statement:

  • Assist with management of Greenleaf Accommodation Services.
  • Support and supervise a team of Support Workers within your allocated houses.
  • Provide role-modelling, mentoring and face to face coaching to the support staff.
  • Support the induction and orientation process for new team members (including site induction and shadow/ buddy shift support).
  • Ensure proper risk assessment is carried out for participants in your allocated houses.
  • Assist in coordinating the day-to-day activities, routines and systems which support the daily living of your participants and the operations of your allocated houses.
  • Assist clients with their day-to-day care arrangements in accordance with shift plans as part of the regular support roster at least once per week in each allocated house, which can include social support, domestic assistance including yard maintenance and transport needs.
  • Develop and implementing high quality Support/ Care/ Recovery plans.
  • Collaborate with participants and relevant stakeholders to gather input for developing person-centered risk assessment, Care/ Support/ Recovery plans and assist participants to work towards achieving their individual goals.
  • Ensure all the participants’ case notes, shift notes, incident reports, communication records, mileage and house files are up to date in the Greenleaf data management system (currently Care Master).
  • Undertake all necessary reporting including compliance reporting (reportable incidents, restrictive practices, etc.) in a timely manner.
  • Deliver a consistent and quality customer service to internal and external stakeholders.
  • Monitor, evaluate and provide feedback on participants’ shift notes, other relevant reports, recording charts and incidents to ensure delivery of quality services.
  • Act as the first point of contact of your participants and houses.
  • Assist and support your participants in their medical/ psychiatric reviews and NDIS Plan reviews.
  • Organise regular house audits to ensure high level of quality service delivery.

Requirements for Appointment:

The successful applicant must have:

  • Experience in providing positive support to clients with challenging behaviour
  • Minimum 1 year experience in managing SIL accommodation (at least 2 houses and their participants and staff).
  • A current NDIS Worker Screening Check
  • A valid National Police Check (issued within 3-month period) or willing to undergo a National Police Check
  • A completion certificate for NDIS Worker Orientation
  • A current first aid certificate
  • A current Australian Driver's Licence
  • Comprehensive car insurance
  • Availability to work Monday to Friday, office hours.

Selection Criteria:

Essential

  • Minimum Diploma in Disability/ Community Services/ Mental Health equivalent
  • 1 year’s demonstrated experience in managing service delivery with disability services - including experience in managing group homes.
  • Sound oral and written communication and customer service skills, where you will have past experience liaising with key stakeholders including families and multi-disciplinary teams.
  • Demonstrated ability to prioritise tasks - share information and manage your time to an excellent standard.
  • Demonstrated strong leadership skills and ability to foster committed and collaborative teams.
  • Demonstrated experience in supervising teams with minimal supervision.
  • Demonstrated working knowledge of the National Disability Insurance Scheme (NDIS) and NDIS Quality & Safeguards Commission.

Desirable

  • Tertiary qualification in Social Work, Psychology, Counselling, etc.
  • Therapeutic Crisis Intervention Training Certificate.
